Currently recharting on a tooth - (eg caries on a tooth that previously had caries on the same surface) causes deletion of the original charting (which might be from years ago) ...which is a fairly serious medicolegal issue - now it appears that the patient medical records have been altered and are incomplete, effectively Dentally has deleted the dentist’s notes and is leaving the dentist open to litigation due to missed diagnosis / poor record keeping, and has made the charting confusing to use and less useful because the old record is absent.

The auto delete function might be useful when charting ...but should perhaps be limited to deleting only recently charted items - ie in the last hour or 24 hours.

Thanks for raising this. Just to clarify how Dentally is designed to work: when you re-chart on the same tooth/surface, the earlier charting is not deleted. The current chart will show the latest state, but the previous entry remains in Charting History (so “what was true at the time” stays in the record). Editing or deleting a new item should not overwrite what was charted previously.

Could you try this on one example and let me know what you see?

  1. Open the patient → ChartCharting History.

  2. Make sure any filters are set to show all items (including completed/cancelled).

  3. Check whether the earlier caries entry is listed there.

If the historic entry truly isn’t present, that’s not expected and I’d like to have our Support team investigate straight away so we can fix whatever’s going on.

 

I’ve also logged your suggestion to limit any auto-clear behaviour to recent items (e.g. last hour/24 hours), and to add clearer signalling when re-charting will update the on-screen chart. I’ll keep this thread updated with what we find and any changes we make. 😊


...I can confirm that the historic charting is absent altogether from the history and base chart tabs, and also from the tooth’s history list when you hover the charting

Our Support team have reached out to explain how to solve this. This wasn’t a platform error. What you saw depends on how the item is applied when re-charting the same surface.

 

Here’s how to re-chart without losing the previous entry on the same surface

  • When adding a new chart item to the same tooth/surface, hold CTRL or ALT while charting.

    This preserves the historic entry and adds the new one, so your history remains intact.

  • If you simply click (without a modifier) on the same surface with the same item, you’ll replace the latest entry for that surface.

 

Tip: To review what’s been recorded, open Chart → Charting History and ensure filters are set to show all items.
