Addition of other items for the root status should be added, such as Apical abscess/infection (PA lesion), and root fracture (VRF). Variations on location of cracks that are more distinct and unique, including options for location i.e. buccal crack lingual crack mesial or distal. Following that thought process having UI that indicates fractures of restorations or parts of teeth. A specific icon/UI change for unsatisfactory restorations or restorations with secondary caries. Even variations to RCT such as overfill or underfill or unsatisfactory. Preferably all three.  I believe these items will help ensure a comprehensive picture of a patients current dental status.

I believe almost everything you’ve listed can already be charted today by combining custom treatment items and the Root Chart view.

1. Create one-click “root-only” items for PA lesions, VRF, over/under-filled RCT, etc.
    •    Go to SettingsTreatments & Plans, add or duplicate a code, then on the Icon tab set:
    •    Region = “Site on tooth” (so it can sit on the root).
    •    Fill Root = “Yes” (this makes the mark appear only when the Root Chart is visible).
    •    Pick an icon shape/colour or switch to Text and use short labels like “PA”, “VRF”, “OF”, “UF”. 
    •    When you enable the Display Roots toggle in the chart settings, these items paint directly on the root; with roots hidden they show a small indicator above/below the tooth so nothing gets missed.

2. Show exactly where a crack or restoration fracture is
    •    Use separate items such as “Crack – Buccal”, “Crack – Lingual” (same setup as above).
    •    For crown-surface cracks or secondary caries, chart the item on the ordinary tooth view; hold Alt/Option (or right-click) to link multiple surfaces if the crack spans more than one

3. Flag an unsatisfactory or failed restoration
    •    Duplicate the existing restoration code, change its icon colour (e.g. red outline) or swap the shape (triangle, star, etc.) so it’s instantly recognisable.
    •    If the restoration also changes the status of the tooth (e.g. crowned implant), set an appropriate Tooth Status in the item’s details, that feeds through to the odontogram, perio chart, reports, etc.


Also, because icon edits apply practice-wide, you can create a small “Diagnostic” category and keep all these special root items together, this way clinicians get clarity without cluttering everyday lists.
