Is this the medical history form in the patient record in Dentally, or what the patients see when using the patient portal, or something else? A screenshot would help! Thank you

Thanks for clarifying ​@MarkBEvans - I have raised feature requests for both easier medical history workflow, and for inclusion of key conditions by default.

Regarding your second request, in the interim please note you can edit the medical history questions yourself, so you could include any conditions you feel are missing, as a workaround to ensure you capture the info you are wanting.
