FP17 DC forms

HI. I am familiar with the FP17forms that the patients sign through dentally. We also need to sign FP17DC. Can these also be done through dentally? I am referring to the form https://www.nhsbsa.nhs.uk/sites/default/files/2025-04/FP17DC%20form%20England%20-%20April%202025.pdf

 

I could not find any reference to it here or in the support portal.

At the moment, FP17DC isn’t supported for digital signing in Dentally.  In practice, most NHS and DPMS practices now use the NHS Treatment Plan Estimate in Dentally instead, which is intended to serve the same purpose as FP17DC. It outlines the proposed treatment, shows the expected NHS charges, and captures patient acknowledgement, and is generally accepted in place of FP17DC.

One of the reasons FP17DC isn’t supported directly is that the form changes regularly, whereas the treatment plan estimate is the more stable, NHS-aligned approach used day to day. If your commissioner or internal processes still specifically require FP17DC, the workaround would be to complete it externally and upload it to the patient record as an attachment. 😊

hi ​@Saymon . Thank you. We had one dental health board ask us to use the FP17DC and that's the reason we asked. We do use the estimate and it works great. I understand now and we will look into uploading it instead.
